Plan Benefits: What are the plan benefits and coverages?
- Identify the plan's copays, deductibles & coinsurances.
- What is the plan's "maximum out of pocket" for the year?
- View the plan's "Summary of Benefits" for detailed plan info.
Network Strength: Are there a lot of doctors and hospitals In-network?
- Does your doctor accept this plan?
- Identify which hospitals accept the plan.
- Does the network extend out of state?
Max-Out-of-Pocket: What is your maximum out of pocket for the calender year?
- All covered expenses go towards your maximum-out-of-pocket which includes copays, deductibles and coinsurances, including prescriptions.
- Once your max-out-of-pocket has been met, the carrier pays 100% of your covered medical and prescription claims for the remainder of the year.
